With your zip code, you will be given the nearest location. At the Secretary of State office you will have to request a Driving Record Abstract request form, a DSD DC-164 form.

If you do require high risk insurance, you may want to speak to your current insurance provider to see if they are a high risk auto insurance company. If they are not or if you have no insurance provider there is a couple of websites can do the comparing for you among different providers classified as high risk auto insurance companies.
